Guardianship

Our Ward-Guardian relationship at Colorado Elder Care Solutions (CECS) is measured by enhanced quality of life and integrity. We adhere, at all times, to the National Guardianship Association Code of Ethics and Standards of Practice as well as Colorado law governing the duties of guardianship.

Our goal in providing guardianship services is to treat each ward as an individual, recognizing their unique needs and wants. We believe that guardianship is more than just making the right medical decision. It involves really listening to our clients and understanding their wants and needs to ensure the highest quality of life. We build a relationship with the physicians to be proactive and assure that advanced directive instructions are in place and followed. Our 24 hour availability means that we are there when any need or change in condition arises.

WHEN IS CARE MANAGEMENT NEEDED?

Any time a person is incapable of recognizing and or meeting their own care needs, Care Management is the solution. This includes times in which family or fiduciaries, with the best intentions, still need someone to help coordinate care between physicians, specialists, and day-to-day needs. Care managers are able to address safety, medication compliance, needed social support systems and assess degree of cognitive impairment. We create a safe and healthy environment for each individual.

WHAT IS CARE MANAGEMENT?

Care Management is the process in which a skilled professional, often times a nurse or social worker, completes an assessment and develops a care plan to guide in the support services of an individual. A care plan includes medical treatment, safety assessment with recommendations, nutritional, social and environmental support for a client.

WHY NURSE CARE MANAGEMENT?

As nurse Care Managers, we are able to use clinical training and expertise to assess the client's current and changing medical conditions. We assess the effectiveness of the current medication regimen and collaborate with the physician to address all of the client's health needs.

HOW CAN CARE MANAGEMENT HELP MY FAMILY?

Often times, members of the same family perceive healthcare decisions differently. The Care Manager supports the family in becoming a cohesive group so that together they can all support the client through challenging times.
