WHEN IS CARE MANAGEMENT NEEDED?Any time a person is incapable of recognizing and or meeting their own care needs, Care Management is the solution. This includes times in which family or fiduciaries, with the best intentions, still need someone to help coordinate care between physicians, specialists, and day-to-day needs. Care managers are able to address safety, medication compliance, needed social support systems and assess degree of cognitive impairment. We create a safe and healthy environment for each individual. WHAT IS CARE MANAGEMENT?Care Management is the process in which a skilled professional, often times a nurse or social worker, completes an assessment and develops a care plan to guide in the support services of an individual. A care plan includes medical treatment, safety assessment with recommendations, nutritional, social and environmental support for a client. WHY NURSE CARE MANAGEMENT?As nurse Care Managers, we are able to use clinical training and expertise to assess the client's current and changing medical conditions. We assess the effectiveness of the current medication regimen and collaborate with the physician to address all of the client's health needs. HOW CAN CARE MANAGEMENT HELP MY FAMILY?Often times, members of the same family perceive healthcare decisions differently.
